Click on Submit a Claim

You will need the following documents ready with you to complete the claim submission process

Required documents

    For Members

    For HR/Broker/LP On Behalf Of Member/Customer

  1. Proof of ID card or Passport*
  2. Medical Certificate/Diagnosis/Doctors Report/Prescription*
  3. Discharge Letter (if admitted to hospital)**
  4. Invoices with detailed breakdown*
  5. Result of Laboratory and Diagnostic Test**
  6. Other Documents

* Mandatory

** Conditionally mandatory if patient was admitted to hospital and/or some medical tests were perfromed

  1. Member Card*
  2. Proof of ID card or Passport*
  3. Medical Certificate/Diagnosis/Doctors Report/Prescription*
  4. Discharge Letter (if admitted to hospital)**
  5. Invoices with detailed breakdown*
  6. Result of Laboratory and Diagnostic Test**
  7. Proof of Customers Bank Account*
  8. Proof of Relationship of Insured and Claimant (In case Insured and Claimant are different person)
  9. Other Documents
